About the Role

As a Nurse Chronic Care Coordinator, you will conduct telephonic encounters with patients and develop care plans using our electronic health record system. This position starts as an Independent 1099 Contractor but has the potential to transition to full-time W2 employment with benefits.

What You'll Do

• Perform telephonic encounters with patients each month.
• Develop detailed care plans for patients and doctors.
• Carry a patient panel of at least 100 patients per month.
• Complete encounters for 90% of assigned patients.
• Participate in regular meetings to discuss work scope.

What We're Looking For

• Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.).
• Current COMPACT license to practice as an RN/LVN/LPN in your state.
• At least two years of clinical experience in Med/Surg, Case Management, or home health care.
• Experience with Electronic Medical Records and Microsoft Office applications.
• Strong initiative, organization, and problem-solving skills.
• High-speed internet and a desktop or laptop computer (Windows or Mac).
• Excellent verbal, written, and listening skills.

Compensation

We offer a productivity-based pay structure:

• $10.00 per completed patient encounter (up to 99 encounters/month).
• $10.25 per encounter (100-149 encounters/month).
• $12.00 per encounter (150-199 encounters/month).
• $14.00 per encounter (200-249 encounters/month).
• $16.00 per encounter (250+ encounters/month).

Payment tier increases require three months of consistency to achieve. Each patient encounter takes a minimum of 20 minutes (time is cumulative).
